These two get confused constantly — partly because some ionizer marketing blends them together. They are different things solving different (claimed) problems.

What the evidence looks like for each

Hydrogen water: molecular hydrogen has a substantial and growing research base — human trials in exercise recovery and oxidative stress markers among others. Findings are promising though not conclusive, and effects vary between individuals.

Alkaline water: the body tightly regulates blood pH regardless of what you drink; stomach acid largely neutralizes alkaline water on arrival. Most sweeping claims made for alkaline water have limited clinical support. Some people simply prefer its taste — a legitimate reason that needs no health justification.

A note on "hydrogen-rich alkaline ionizers"

Water ionizers do produce some dissolved hydrogen as a side effect of electrolysis, which is where marketing blurs the line. But concentration is the key variable — dedicated hydrogen devices are designed to maximize dissolved H2, while ionizers optimize for pH. If dissolved hydrogen is your goal, buy a device built for it.

Bottom line

If you're choosing between the two based on the research: molecular hydrogen has the substantially deeper evidence base. If you just prefer the taste of alkaline water, that's fine too — but don't pay ionizer prices for hydrogen benefits an ionizer isn't designed to deliver.
